ezEMRx offers a web-based Electronic Health Record and Practice Management application to clinics (Public Health & Private Practice) and their authorized users using secure data centers. ezEMRx is dedicated to protecting the privacy of the users of our products, including the health information stored or transmitted through our products. ezEMRx's products are developed with the highest levels of security.

Patients can access their health information available through ezEMRx websites at your request. The agreement relating to ezEMRx products, and its usage takes precedence over this Privacy Policy.

 

Collection of Personal Information

While visiting ezEMRx, the IP address used to access the site will be logged along with the dates and times of access. This information is purely used to analyze trends, administer the site, track movement, and gather broad demographic information for internal use. Most importantly, all recorded IP addresses are not linked to personally identifiable information.

Permitted Use of Information

ezEMRx may allow other health care providers and their business associates to access to provider/patient information for treatment. This includes disclosing health information through selected applications such as clinical laboratory reporting, billing and e-prescribing, to other health care providers.  Protected health information, agreed to be disclosed, is also provided to other health providers for treatment.

You agree to be bound by the terms of any licensing and other agreements relating to such third-party products and services and allow access to health information to your patients by logging into the ezEMRx Patient portal. The Health Care provider controls the patient’s access to their health records, and the type of information they can see.

 

Account Audits

ezEMRx performs audits on all events. System use is logged and audited to ensure that users are using the system appropriately. If we have questions about your use of the system, we may contact you. We may also disclose your identity to others to assist in the investigation of suspected misuse of our products, and otherwise to ensure the proper operation of our products.

 

Security

You are solely responsible for (1) maintaining the strict confidentiality of any user name or password assigned to you (known as credentials), (2) preventing others from using your credentials to access ezEMRx, (3) any damages or losses that might be incurred or suffered as a result of your failure to maintain the strict confidentiality of your credentials, and (4) informing ezEMRx in writing of any need to deactivate your credentials due to potential or actual security breaches. ezEMRx is not liable for any harm related to the theft of your credentials, your disclosure of your credentials, or your authorization to allow another person or entity to access and use the site using your credentials. You agree to immediately notify ezEMRx, in writing, in case of any unauthorized use of your credentials.
